The Accounting of Disclosures Report

Organizations that use Protected Health Information (PHI) are required by HIPAA to provide patients (upon request) a list of disclosures of the patient's PHI. This means that your organization must keep track of these disclosures and be able to compile the necessary information about them into a single report to patients.
